Subject: Header rearrangement, move inspircd.h to top, remove stdio, stdlib,
 stdblahblah that we dont even use, remove redeclared <string>, <map>,
 <vector>. This (1) fixes the irc::string::move warnings on vc8 (2) speeds up
 compile a lot
